Cardiogenic Shock

  • Treatment

    • ABCs
      • O2
      • IV
      • EKG
    • Chest pain
      • IV NTG 5-100 mcg/min
      • or morphine sulfate 2 mg increments
    • Inotropes
      • moderate hypotension: dobutamine 2.5-20 ug/kg/min + dopamine 2.5-20 ug/kg/min titrated to effect
      • severe hypotension: Norepinephrine 2ug/min titrated to effect
    • Afterload reduction
      • intraaortic balloon pump (contraindicated in severe aortic regurgitation)
      • if acute mitral regurgitation: sodium nitroprusside 0.5-10 ug/kg/min + dobutamine 2.5-20 ug/kg/min
    • Tx underlying cause
      • PCI
      • fibrinolysis if PCI unavailable
