      • neurologic checks q 4 hours
      • No IM injections if platelets < 50,000
      • Oral care q shift
    • Diet: No Unpeeled Fresh Fruits or Fresh Vegetables
    • Activity: Encourage Activity with Assistance, Fall Precautions
    • Labs: CBC with plts, Chem qday
    • IV: None If Adequate Oral Intake
    • Special: oncology consult
    • Medications:
      • Filgrastim (G-CSF) 300 mg SQ qd (increase dose to 480 mg for patients > 70 kg)
      • antiemetics
        • Ondansetron (Zofran) 16 mg PO prechemotherapy
        • Dexamethasone 12mg IV pretherapy, 8 mg PO qd days 2-4
      • prn antiemetics
        • Prochlorperazine (Compazine) 10 mg IV/PO q 6 hours prn nausea
        • Metoclopramide (Reglan) 10 mg IV/PO q 6 hours prn nausea
        • Dronabinol (THC or Marinol) 2.5-10 mg PO q 6 hours prn
        • Lorazepam (Ativan): 0.5-2mg IV/PO q 6 hours prn nausea
      • Magic Mouthwash (viscous lidocaine + Benadryl + Maalox) 15 mL swish and spit q 2 hours prn mouth pain
      • Cetacaine spray prn mouth pain
      • Baclofen 5-10 mg PO of 8 hours prn hiccups
      • Docusate 250 mg PO bid and/or senna 2 tablets PO qhs
      • Bisacodyl 5-10 mg PO qd prn constipation
    • Call HO: temp > 37.9 C
  • Discharge

    • criteria: ambulating, able to keep up with nutritional support, afebrile
