Asthma chronic visit

    • dyspnea
    • cough
    • wheezing
    • frequency of symptoms (3x per week = mild, daily = moderate, throughout the day = severe)
    • frequency of albuterol usage (3x per week = mild, daily = moderate, several times per day = severe)
    • night time awakenings (3-4x per month = mild, 4+ per month = moderate, nightly = severe)
    • activity limitation (minor = mild, some = moderate, extreme = severe)
    • number of asthma hospitalizations needing steroids (2 = mild)
  • Px

    • pulm, wheezing
    • ENT (pale swollen nasal mucosa, nasal polyps)
  • Labs

    • FEV1: 60-80% = moderate, < 60% = severe
    • FEV1/FVC: below normal = moderate
  • Classification

    • intermittent, mild persistent, moderate persistent, severe persistent
    • anything less than mild is intermittent
  • Tx

    • albuterol prn
    • low dose ICS for mild persistent
    • medium dose ICS for moderate persistent
    • high dose ICS + LABA for severe persistent
